Once checkout is completed, please fill out the Custom Order Form (Black Jersey Order Form) (Gray Jersey Order Form) and email it to insidesales@valleyfashions.com.

Please make sure to provide the following when emailing the order form:

• Name
• Shipping Address
• Jersey Type (i.e. long sleeve or short sleeve)
• Color (i.e. black or gray)
• Order Number

Please note, your jersey order will not be processed if the information required (i.e. order form, name, shipping address, etc.) is not complete in the email.
